The two provinces are next to each other, so there is no need to go back to Hanoi between them. The Ha Giang to Cao Bang road journey is about 300 km and takes 7 to 8 hours by limousine van, or up to 10 hours with stops. Direct vans leave in the morning only, there are no night buses on this route, and tickets start from around 350,000 VND per person.

Vans run from Cao Bang to Ha Giang on the same road and take about the same time. Schedules change by season and operator, so check the departure time one or two days before you travel. A private car costs more but lets you stop for photos along the way.
